Dr French has personally provided medical records to you or your new provider since his retirement.
He will continue to assist with these requests until he is able, however IRON MOUNTAIN has custodianship of your records as well. They, however may take up to 30 days to deliver your records.
Dr. French has paid for their services up front and you will not be charged for record requests.

TO OUR FORMER BCBS HMO PATIENTS:
For those of you who did not clarify/specify your desired PCP, Guidehealth automatically assigned Dr Ryan Cook as your PCP on 7/31/2024. A letter was sent from Guidehealth outlining this procedure in April 2024.
STARTING 8/5/2024, IF YOU NEED TO REACH GUIDEHEALTH TO CLARIFY YOUR PCP:
Go to secure.guidehealth.com and go to contact page and create a short inquiry and you will receive a "ticket" and your request willl be handled promptly.
Although Dr. French was an independent physician, many of his patients, especially with HMO's are attributed to Amita Health CIN (AHCIN) and are managed within the Ascension system (formerly called Amita or Presence). Although Ascension Illinois will sell several of its hospitals to Prime Healthcare in 2025, they will NOT sell Ascension St. Joseph Hospital Chicago nor its medical groups, which is good news. There are many fine physicians within this system:
